Case Management helps seniors combat loneliness and isolation while helping them age safely in place. Our professional services give seniors the support and plan they need to thrive.

The case management program provides advocacy and individualized support to help our clients maintain their health, independence and quality of life.

Case managers provide compassionate support through personalized care planning and connection to critical resources to support our clients’ physical, emotional and social wellbeing.  Through proactive outreach and ongoing follow-up, case managers help seniors navigate the complex challenges such as accessing healthcare, securing affordable housing, access to community resources and reducing social isolation. 

Our case management program is available to adults (60+) who reside in Collier County or southern Lee County. 

Our experienced case managers help clients gain access to:

  • Affordable housing                   

  • Dental and medical care   

  • SNAP benefits

  • Medicaid/Medicaid LTC     

  • Emergency preparedness planning            

  • Healthcare system benefits and entitlements  

                                                        

  • Home delivered meals

  • Home health services

  • Rental assistance

  • Healthcare

  • Budgeting assistance

  • Transportation to medical appointments (paratransit services)

Cognitive Health
Our Cognitive Health Center offers structured groups to help with early memory loss, as well as mild to moderate stage dementia.

Mental Health Counseling assists seniors and their families with challenges brought about by life transitions such as aging, cognitive decline, grief, depression and anxiety.
